Cardiac Teaching Program - Nurse Home Visit

Our Cardiac Teaching Program offers support and education to heart attack patients and their families during the first 6 weeks after discharge from Hospital. It is a continuation of education that starts while the heart attack patient is still in Hospital.

The visiting Nurse meets with the patient and family 7 - 10 days following discharge from Hospital. Information received while in Hospital is reinforced and patients have an opportunity to clarify any concerns that may have arisen after discharge. The Nurse reviews detailed information on:

  • cardiovascular health
  • diet
  • exercise
  • medications
  • Physician appointments

One month after discharge, the patient and family attend an evening class and meet with other individuals who have suffered a heart attack. They receive further information on cardiovascular health aimed to help them adapt healthier lifestyle behaviours.
