Discharge Planning

To provide the most effective care for you, it is necessary to plan for discharge early in your stay. When community services are needed, they will be planned in collaboration with patient and family. The discharge time is 1100 hours (11:00 a.m.). Hospital staff will assist you in arranging any services that you require.

Family members are an important part of the discharge plan. If necessary, community services will be planned in co-operation with family support. Some patients require other types of care. Should it be determined that this care can be provided better by another facility, our staff will make all the necessary plans for your transfer.

If returning home is not possible and if further rehabilitation is not required, planning for long term care or alternate housing will begin. Our staff will help you make the best choice to suit your particular needs. We encourage patients to go home temporarily with services from Home Care to await Personal Care Home placement.  For further information on the Personal Care Home program, please click here.

If it has been decided that you are ready to be discharged or need to be transferred to another facility and you refuse this discharge plan, you will then be responsible for the full cost of Hospitalization under the regulations of the Health Services Insurance Act.
