Home Care Program

The Home Care program provides essential supports to persons, regardless of age, whom require health services or assistance with daily living.  As long as it remains safe, the Home Care program augments the resources of family and community to support people living in their homes. This is done through promotion of care, management of health issues and facilitation of hospital discharges. The program also provides comprehensive palliative care services and alternatives to entering a personal care home and/or Long Term Care Facilities.

The WRHA hospital based Home Care Program has Case Coordinators located on site. They are members of the interdisciplinary team at Concordia Hospital. Case coordinators assess a client's functional needs and complete service planning prior to discharge.
