Patient & Health Care Provider Rights and Responsibilities
Developed by the Patient Care Teams with input from patients and families. Concordia Hospital is dedicated to providing quality care and service.
Our Patient Bill of Rights outlines the rights and responsibilities Patients have regarding their health care. It also describes the partnership between Patients and Health Care Providers in ensuring these rights are achieved.
Patient has the right to:
- Receive quality care and services within the capabilities and policies of the facility
- Receive easily understood information about diagnostic test results, health care plan, and available care options
- Be treated with courtesy, dignity and respect
- Privacy while receiving care within the capabilities of the facility
- Confidentiality of information
- Expect that efforts will be made to respect religious and cultural freedom
- Exercise freedom of expression and choice regarding consent, complaints, and decisions about the care plan
- Have access to resources for solving disputes, grievances, and conflicts
- Have honoured their wishes as outlined by the Patient directly or through the Health Care Directive
Patient has the responsibility to:
- Discuss health care concerns clearly with the health care provider
- Participate in health care planning and self care
- Practice and support recommended safety measures
- Treat health care providers with courtesy, dignity and respect
- When able assume responsibility for their own health and wellness
- If available, provide the facility with his/her Health Care Directive
Health Care Provider has the right to:
- Have access to patient health information needed for the planning of care for the patient
- Work in a safe environment that promotes team work, quality improvement, and achievement of professional standards
- Be supported in carrying out professional health care responsibilities based on the facility's mission and values
- Receive compassionate support through stressful experiences
Health Care Provider has the responsibility to:
- Respect patient rights and responsibilities
- Share information in an honest, complete and timely manner without breaking confidentiality
- Demonstrate accountability for professional care standards
- Treat individuals with fairness, respect and dignity
- Collaborate with family, designated others, and other health care providers in the planning and management of care
- Participate in quality improvement initiatives
- Provide an environment which promotes dignity, peace and comfort at the time of death
