Your Health Record

Each time you are a patient at Concordia Hospital, information is added to your health record. All information is handled in a secure and confidential manner in accordance with the Personal Health Information Act of Manitoba.

If you would like a copy of your health record or if you would like to view your health record after you have been discharged, contact  Health Records at 661-7164. Access to your health information can only be authorized by yourself or your personal representative. Health Record staff will require you to sign an Access to Personal Health Information form if you wish a copy of your record or a viewing.

A Health Information Technologist is available to view your record with you to assist you with reading the entries.
